Olive oil and oxidative stress | What polyphenols really achieve

Polyphenols are among the scientifically most interesting minor components of high-quality extra virgin olive oils.

And there is even an expressly authorized European health claim for one specific connection:

Olive oil polyphenols contribute to the protection of blood lipids from oxidative stress.

This is not a general advertising formula, but a regulated EU health claim with clear prerequisites. At the same time, randomized human studies and a meta-analysis show that phenol-rich olive oils can show measurable advantages over lower-phenol comparison oils regarding individual biomarkers of oxidative processes.

That is precisely why a precise classification is worthwhile.

Reactive oxygen species are not simply enemies of our body. They arise during normal metabolism and take on important tasks in cellular signal transduction. Scientists only speak of oxidative stress when pro-oxidative processes overwhelm redox regulation, protection, and adaptation systems.

The exciting question is therefore not whether olive oil shields every cell like a protective barrier. The exciting question is: Which positive effects have actually been measured in humans, which statement is legally permitted, and where does the evidence end?

Olive oil polyphenols and oxidative stress | The short answer

Are olive oil polyphenols scientifically relevant? Yes. Randomized human studies and a meta-analysis show positive changes in individual biomarkers of oxidative processes when comparing higher-phenol olive oils with lower-phenol ones.
What did the 2023 meta-analysis show? In eight randomized clinical trials, oxidized LDL, malondialdehyde, and the antioxidant capacity of plasma were investigated, among others. In the overall evaluation, higher-phenol olive oils were associated with significantly lower levels of oxidized LDL and malondialdehyde; the MDA subgroup analysis put the certainty of this result into perspective. No significant difference was found for FRAP.
Is there an authorized EU health claim? Yes. For olive oil that meets the legal requirements, it is authorized to state that olive oil polyphenols contribute to the protection of blood lipids from oxidative stress.
Does this apply to every olive oil? No. The claim requires at least 5 mg of hydroxytyrosol and its derivatives per 20 g of olive oil. In addition, the daily intake of 20 g must be pointed out for the positive effect to occur.
Does more total polyphenol automatically mean more health impact? No. The total polyphenol value, individual phenolic compounds, measurement method, dosage, and clinical endpoint must be kept separate.

Oxidative stress | What is scientifically meant by this

Reactive oxygen species are constantly being created in our body. The English abbreviation ROS | Reactive Oxygen Species | is frequently used for this.

The simplified concept is often:

Free radicals are bad | Antioxidants are good.

Modern redox biology does not work that simply.

Certain reactive oxygen compounds are part of normal biological processes. Among other things, they act as signaling molecules and are involved in the regulation of various cell functions.

It can become problematic when pro-oxidative processes overwhelm existing regulation, adaptation, and repair systems.

Then, redox signals can be disrupted and biomolecules can be oxidatively modified.

The term oxidative stress therefore does not describe a single molecule, nor a condition that one can simply "switch off" with a spoonful of a food item.

This is crucial for the classification of polyphenols.


What are polyphenols in olive oil?

Polyphenols are not a single substance, but a large group of plant compounds.

In extra virgin olive oil, hydroxytyrosol, tyrosol, as well as various compounds derived from oleuropein and ligstroside are part of the phenolic profile.

Oleocanthal and oleacein are also particularly well-known.

I find this distinction important because simplified texts often pretend as if "polyphenol" were already a precise analytical description.

It is not.

Also the statement:

"This olive oil has 800 mg of polyphenols per kilogram."

does not yet answer all important questions.

One would additionally need to know:

  • Which compounds were measured?
  • With which method was it analyzed?
  • Which reference system was used?
  • Which harvest was investigated?
  • To which batch does the analysis belong?
  • When was the sample analyzed?

Exactly for this reason, a total polyphenol value is not the same as a complete phenolic profile.

Human studies | Which positive effects were actually measured?

Here, the evidence is stronger than the blanket phrasing "antioxidants in olive oil" would suggest.

A systematic review published in 2023 with a dose-response meta-analysis evaluated eight randomized clinical trials in which phenol-rich and phenol-poor olive oils were compared.

The meta-analysis found in phenol-rich olive oils:

  • significantly lower values of oxidized LDL | ox-LDL
  • in the overall evaluation, significantly lower values of malondialdehyde | MDA, although a subgroup analysis put the certainty of this finding into perspective
  • no significant difference for FRAP as a measure of the ferric reducing ability of plasma

In the dose-response analysis, a significant linear relationship was also shown between the phenol content of the olive oil and oxidized LDL.

This is a reliable positive statement: The phenol content of olive oil can measurably influence individual markers of oxidative processes in controlled human studies.

At the same time, the statement does not apply without limits. The studies differed in the olive oils used, phenol levels, groups of people, study duration, quantity, and measurement methods. Also, not all biomarkers investigated were equally altered.

Furthermore, an alteration of a biomarker is not automatically synonymous with a general clinical effect on the entire organism.

The positive evidence is real. Its reach must nevertheless fit the investigated endpoint.


The EU health claim | The strongest established statement on olive oil polyphenols

For olive oil polyphenols, there exists an expressly authorized health claim in the European Union.

It reads:

Olive oil polyphenols contribute to the protection of blood lipids from oxidative stress.

However, this statement may not be applied across the board to every olive oil.

The claim may only be used for olive oil that contains at least:

5 mg of hydroxytyrosol and its derivatives per 20 g of olive oil.

Additionally, it must be communicated that the positive effect occurs with a daily intake of:

20 g of olive oil.

This condition is crucial.

Mathematically, 5 mg in 20 g corresponds to 250 mg per kilogram.

But this must not be turned into the simplified rule:

250 mg of any total polyphenols per kilogram = EU health claim met.

The legal provision refers to certain relevant compounds.

A high total polyphenol value alone therefore does not automatically prove that a specific product meets the requirements of the health claim.

Green Agora never treats a health claim as a permanent property of a product name.

If we publish a specific health-related claim about a product, the following must match:

  • Product
  • Harvest
  • Batch
  • Analysis
  • Analytical method
  • Date
  • Current legal situation

How far the EU health claim reaches | And where its limit lies

The authorized statement refers to blood lipids and oxidative stress.

It does not mean:

  • that olive oil protects every body cell from oxidative stress
  • that olive oil stops the aging process
  • that an olive oil prevents wrinkles
  • that polyphenols generally reduce inflammation throughout the body
  • that a phenol-rich olive oil automatically lowers blood pressure
  • that a high polyphenol value automatically improves cholesterol levels

Research question and authorized product statement are two different levels.

I would like to make this distinction consistently visible at Green Agora.

What about anti-aging, skin, and collagen?

Especially here, overly grand promises are made very quickly.

There are laboratory studies with cell models for individual olive ingredients.

Oleuropein, hydroxytyrosol, and related compounds are investigated, for example, regarding various biological processes.

Such work is scientifically interesting.

But:

An investigation on cultured human skin cells is no proof that the consumption of a certain olive oil in humans improves the collagen structure or prevents skin aging.

Studies with externally applied extracts also answer a different question than studies on the consumption of olive oil.

Therefore, I would not present high-quality olive oil as an anti-aging treatment.

I find reality more interesting.

Olive oil is a complex food.

Its fatty acids, phenolic compounds, flavorings, and other ingredients have been intensively studied for many years.

That deserves scientific curiosity.

There is no need for an exaggerated story of efficacy.


Classify polyphenol content correctly | More is not automatically more effect

A number can be fascinating.

500 mg.

1,000 mg.

2,000 mg per kilogram.

But a larger number does not automatically answer the question of which olive oil fits a person better.

A total polyphenol value initially says something about an analytically determined quantity.

It does not automatically say:

  • which individual phenolic compounds are present
  • how high the oleocanthal level is
  • how high the oleacein level is
  • whether the conditions of the EU health claim are met
  • how the oil is perceived sensorially
  • whether a person likes using this olive oil

Exactly here, the different worlds of Green Agora collide for me.

And the person in front of the bottle.

A very intense oil with pronounced bitterness and pungency can be fascinating for one person and too dominant for another in daily use.

Intensity and quality are not the same thing.


What does the polyphenol content of an olive oil depend on?

The phenolic character of an olive oil does not only emerge in the bottle.

It is influenced by many factors.

  • Olive variety | Different cultivars can develop different phenolic profiles.
  • Ripeness and harvest time | The composition of phenolic compounds changes during fruit ripening. An early harvest can be associated with higher concentrations of certain phenols, but is not an automatic quality guarantee.
  • Processing | Time between harvest and processing, crushing, malaxation, temperature, oxygen contact, and other process parameters can influence the final profile.
  • Storage | Light, heat, oxygen, and time can change the oil and the content of individual phenolic compounds.
  • Analysis time | A lab value always belongs to a specific sample at a specific point in time.

An analytical value is not a brand's tattoo.

It is a snapshot of a natural product.

Extra virgin olive oil | The quality base and what else counts

Extra virgin olive oil is a legally defined quality grade.

Chemical and sensory criteria apply to this.

That is important.

But here too applies:

The quality grade is the beginning | not the complete quality profile of a bottle.

Two oils can both legally be extra virgin olive oil and still differ significantly in:

  • Olive variety
  • Origin
  • Producer
  • Harvest
  • Sensory profile
  • Phenolic profile
  • Freshness
  • Processing
  • Storage

How do you recognize good olive oil?


Must phenol-rich olive oil always be used cold?

Here too, I frequently read a rule that is too simple:

Polyphenols are heat-sensitive | therefore, high-quality olive oil should only be used cold.

I would not formulate it so generally.

Under thermal stress, phenolic compounds can change and their concentrations can decrease.

How strongly this happens depends, among other things, on:

  • Temperature
  • Duration of heating
  • Cooking method
  • Base oil
  • Composition of the oil
  • The food being cooked with it

The scientific literature does not show a simple all-or-nothing model here.

Extra virgin olive oil is therefore not exclusively an oil for salads or pure enjoyment.

Anyone who wants to experience a particularly complex aroma and as much of the original sensory profile as possible can deliberately use a high-quality oil cold or only at the end of a dish.

However, that is a culinary and product-specific decision | not a general ban on heating.

How I view an olive oil at Green Agora

When someone shows me a bottle and asks:

"Ayhan, is this a good olive oil?"

then I don't want to hear a marketing phrase first.

I want to understand.

Which olive variety?

How does the oil smell?

How do bitterness and pungency relate to each other?

What remains in the finish?

What analyses are available?

To which harvest and batch do these analyses belong?

Laboratory values | What they can really show about an olive oil

If a current laboratory report shows a high value of certain phenolic compounds, that is valuable information for me.

It allows me to better understand the analytical profile.

I can think of it in conjunction with sensory characteristics, variety, harvest, and processing.

I can explain differences between oils more precisely.

What I should not do:

Automatically derive a medical effect from a large number.

Old analytical values can be historical information.

A current health-related statement requires a currently relevant basis.


My conclusion | Polyphenols are scientifically interesting and positively verifiable

The most important insight of this article for me is positive.

Olive oil polyphenols are not just a marketing term.

Randomized human studies show measurable effects on individual biomarkers of oxidative processes. A meta-analysis found lower levels of oxidized LDL for phenol-richer compared to phenol-poorer olive oils, and in the overall evaluation also of malondialdehyde; for MDA, a subgroup analysis relativized the certainty of the finding.

And the European Union expressly permits the following statement for olive oils that meet the established conditions:

Olive oil polyphenols contribute to the protection of blood lipids from oxidative stress.

That is a strong statement. Precisely for this reason, it should be used accurately.

Total polyphenol value is not automatically the amount relevant for the claim. A laboratory finding belongs to a specific sample, harvest, batch, and measurement method. And results on individual biomarkers do not justify sweeping statements about anti-aging, disease prevention, or general protection of every body cell.

For me, this precision does not make olive oil any less interesting.

On the contrary.

We do not need to make the positive properties larger than research allows. But we should not make them smaller either.

The more closely we look, the more interesting the bottle becomes.

Do olive oil polyphenols protect against oxidative stress?

For suitable olive oils, there is an approved EU Health Claim: Olive oil polyphenols contribute to the protection of blood lipids from oxidative stress. The claim only applies if the legal conditions are met.

What is scientifically proven about olive oil polyphenols?

Randomized human studies and a meta-analysis show positive changes in individual biomarkers for phenol-richer compared to phenol-poorer olive oils. Oxidized LDL, malondialdehyde, and other markers of oxidative processes were specifically studied.

How much hydroxytyrosol does an olive oil need for the EU health claim?

At least 5 mg of hydroxytyrosol and its derivatives per 20 g of olive oil. Additionally, information must be provided that the positive effect is achieved with a daily intake of 20 g of olive oil.

Are 250 mg of total polyphenols per kilogram automatically enough for the health claim?

Is an olive oil with 2,000 mg/kg of polyphenols automatically healthier than one with 500 mg/kg?

This cannot be derived from the total value alone. Crucial factors include the measured compounds, measurement method, dose, product, harvest, batch, and the specific health endpoint.

Does an antioxidant effect automatically mean anti-aging?

No. Biomarkers of oxidative processes, cell models, and approved health claims are different levels of evidence. No sweeping anti-aging effect of a particular olive oil can be derived from this.

Can extra virgin olive oil be heated even though it contains polyphenols?

Yes. Heating can alter phenolic compounds and reduce their concentration, but this does not lead to a general ban. Temperature, duration, cooking method, and the base oil influence the result.
