Is olive oil healthy? | What research and EU claims actually show

One of the most common questions about olive oil is:

Is olive oil healthy?

I understand why people would like a simple answer to this.

Yes or no.

Good or bad.

Healthy or unhealthy.

But that is not how nutrition works.

First and foremost, olive oil is a food product and, at the same time, one of the characteristic sources of fat in the Mediterranean diet.

It consists primarily of fat, typically contains a high proportion of unsaturated fatty acids, and can contain varying amounts of natural accompanying substances depending on the variety, harvest, processing, and storage.

These include in particular:

  • phenolic compounds
  • tocopherols
  • squalene
  • pigments
  • other natural accompanying substances

And this is precisely the point at which the question becomes scientifically interesting.

Because we have to distinguish between:

  • the general composition of olive oil
  • the quality of a specific olive oil
  • results from observational studies
  • results from randomized nutritional studies
  • laboratory analyses of a specific product
  • authorized health claims of the European Union
  • and statements that cannot simply be transferred scientifically or legally to a single bottle

For me, a serious answer therefore does not begin with an advertising promise.

It begins with the question: What do we actually know?

The good news is:

Today, we know much more than just that olive oil is a component of Mediterranean cuisine.

There are large observational studies, randomized nutritional studies, systematic reviews, and two specifically authorized European health claims that may be relevant for suitable olive oils or corresponding fat profiles.

At the same time, an important boundary remains:

Studies on dietary patterns or population groups are not proof of efficacy for an individual Green Agora bottle.

Is there positive human research on olive oil? Yes. Observational studies, randomized nutritional studies, and meta-analyses show interesting positive correlations. However, the significance depends on the respective study design.
Is there an authorized EU claim for olive oil polyphenols? Yes. The claim is tied to a specific minimum quantity of hydroxytyrosol and its derivatives per 20 g of olive oil.
Is there an authorized EU claim for oleic acid? Yes. It refers to the replacement of saturated fatty acids with unsaturated fatty acids and requires a high content of unsaturated fatty acids.
Is every olive oil the same? No. Variety, harvest, processing, age, sensory profile, fatty acid profile, and phenolic composition can differ significantly.
Are many polyphenols automatically better? High values can be analytically extraordinary. However, total polyphenols are not a sole quality ranking and are not automatically synonymous with the EU claim.
Do you have to drink olive oil pure? No. Olive oil can be used quite normally within meals. Tasting it pure is a matter of enjoyment and sensory preference.
Can olive oil replace therapy? No. Olive oil is a food product. Medical diagnosis, treatment, and therapy must be kept separate from this.

Not all olive oil is the same

When someone asks me if olive oil is healthy, I first want to know:

Which olive oil are we even talking about?

Because there can be significant differences between different products.

For example, in terms of:

  • olive variety
  • harvest time
  • degree of ripeness
  • processing
  • storage
  • age
  • sensory quality
  • fatty acid profile
  • phenol profile

The term olive oil alone does not tell the whole story.

That is why, at Green Agora, we focus particularly intensively on:

Extra virgin olive oil.

This category has clearly defined chemical and sensory requirements.

Nevertheless, it does not mean that every extra virgin olive oil has an identical composition or tastes the same.

What fatty acids does olive oil contain?

Olive oil consists primarily of fat.

What is particularly characteristic is the high proportion of:

monounsaturated fatty acids.

A particularly important one among these is:

oleic acid.

The exact fatty acid composition varies depending on:

  • olive variety
  • region
  • climate
  • harvest year
  • degree of ripeness

However, when it comes to communicating health benefits, it is important to note:

One must not derive just any health effects from the mere presence of oleic acid.

However, there is a specifically authorized European claim for suitable food products.


The authorized EU claim for oleic acid

The authorized claim is:

"Replacing saturated fats with unsaturated fats in the diet contributes to the maintenance of normal blood cholesterol levels. Oleic acid is an unsaturated fat."

This sentence is clearly positive.

However, it has a specific prerequisite.

It may only be used for food products that meet the conditions for:

a high content of unsaturated fatty acids

To qualify, at least:

70 percent of the contained fatty acids must come from unsaturated fatty acids.

Additionally, these unsaturated fatty acids must provide more than:

20 percent of the energy of the food.

This is a good example of how powerful positive nutritional communication can be when the prerequisites are actually met.

Polyphenols | One of the most exciting areas for premium olive oil

When I talk about premium olive oil, I very quickly arrive at:

phenolic compounds.

In virgin olive oil, different compounds and concentrations can occur depending on the product.

These include, for example:

  • hydroxytyrosol and corresponding derivatives
  • tyrosol and corresponding derivatives
  • oleocanthal
  • oleacein
  • other secoiridoids
  • flavonoids

Therefore, I am not just interested in a large number on an analysis sheet.

I want to know:

  • What was analyzed?
  • Using which method?
  • Which harvest is the olive oil from?
  • Which individual compounds were determined?
  • What total values are available?
  • Which sample or batch does the analysis belong to?

Analysis values are valuable.

They become particularly meaningful when their analytical basis is transparent.

Polyphenols in olive oil | Taste, analysis values, and EU health claim


The EU health claim for olive oil polyphenols

There is a specifically authorized health claim for olive oil polyphenols in the European Union.

The official wording is:

"Olive oil polyphenols contribute to the protection of blood lipids from oxidative stress."

This sentence may not simply be used for just any olive oil.

The prerequisite is precisely defined.

The specific olive oil must contain at least:

5 mg of hydroxytyrosol and its derivatives per 20 g of olive oil.

The European regulation mentions, for example:

  • oleuropein complex
  • tyrosol

Furthermore, consumers must be informed that the positive effect is achieved with a daily intake of:

20 g of the corresponding olive oil.

The words "premium," "early harvest," or "high phenolic" do not decide the admissibility of this claim.

The decisive factor is the product-specific analytical requirement.


Total polyphenols and EU claim | Do not equate them

A common mistake is to equate a general total polyphenol value directly with the legal claim conditions.

That is not accurate.

If a laboratory, for example, reports:

800 mg of total polyphenols per kg

this number initially says something about the respective measured sum of substances and the method.

The EU claim, on the other hand, specifically requires:

hydroxytyrosol and its derivatives per 20 g of olive oil.

Therefore, in a product-specific examination, one must consider:

  • which compounds were measured
  • which method was used
  • which specific sample was examined
  • what amount of the relevant compounds is present per 20 g

A spectacular total value may be called spectacular.

However, it does not automatically replace the analysis relevant to the claim.


What does human research say about olive oil?

The positive scientific data situation regarding olive oil does not only consist of laboratory tests.

There are also:

  • large prospective cohort studies
  • randomized nutritional studies
  • systematic reviews
  • meta-analyses

This evidence is important and may be clearly stated.

At the same time, every study must be read according to its design.

An observational study shows correlations.

A randomized nutritional study can allow for stronger causal statements about the intervention studied.

However, a study on a Mediterranean diet with extra virgin olive oil examines:

an entire nutritional concept and not the effect of a single commercial bottle.


PREDIMED | One of the most important randomized nutritional studies

One of the best-known studies in this context is:

PREDIMED.

The revised analysis published in 2018 included:

7,447 participants aged 55 to 80 with a high cardiovascular risk, but without pre-existing cardiovascular disease at the start of the study.

The study compared:

  • Mediterranean diet supplemented with extra virgin olive oil
  • Mediterranean diet supplemented with nuts
  • a control group with a recommendation for fat reduction

The median observation time was:

4.8 years.

The combined primary endpoint consisted of:

  • heart attack
  • stroke
  • death from cardiovascular causes

In the revised analysis, the hazard ratio compared to the control group was:

0.69 for the Mediterranean diet with extra virgin olive oil.

In this study, this corresponds to a significantly lower event rate in the intervention group.

This is a strong randomized result.

But the correct interpretation is:

A Mediterranean diet intervention supplemented with extra virgin olive oil was studied.

Not:

A specific bottle of olive oil lowers an individual's personal risk of disease by exactly 31 percent.

For transparency, it should also be noted:

The original 2013 report was retracted due to randomization issues and republished in 2018 with revised analyses.

The positive results for the primary endpoint remained in the revised analysis.


Large observational study | More olive oil and lower mortality

A large US study from 2022 examined:

92,383 women and men

for up to:

28 years.

During the follow-up, the following were documented:

36,856 deaths.

People with an olive oil consumption of more than:

7 g per day

had a hazard ratio of:

0.81 for all-cause mortality

compared to individuals who never or rarely used olive oil.

Lower statistical risks were also observed for several individual causes of death.

In substitution models, the replacement of:

  • butter
  • margarine
  • mayonnaise
  • dairy fat

with olive oil was associated with more favorable mortality rates.

However, it is also interesting that:

When comparing olive oil with other vegetable oils together, no corresponding significant advantage was shown.

This is scientifically important because it shows:

The question is often not only whether olive oil is used, but also which fat source is replaced by it.

And even more importantly:

This study was observational.

It shows strong correlations, but no definitive causality.


Meta-analyses on olive oil and long-term health data

Meta-analyses of prospective cohorts also show a consistent positive pattern.

A meta-analysis with:

13 prospective cohorts

found a relative risk for higher olive oil consumption compared to lower consumption of:

  • 0.85 for cardiovascular disease
  • 0.83 for all-cause mortality

The dose-response analysis additionally showed that the relationship did not simply continue linearly at ever-increasing amounts.

Above approximately:

20 g per day

only small additional or no further statistical risk reductions were observed.

A medical dosage rule should not be derived from this either.

The number describes the pattern of this aggregated observational data.


Broad systematic review | More than 700,000 people in the mortality analysis

Another systematic review and meta-analysis examined various long-term endpoints with olive oil consumption.

For the analysis of all-cause mortality, data from:

733,420 participants

with:

174,081 deaths

was available.

There, too, a favorable connection was shown overall between higher olive oil consumption and several long-term health outcomes.

Such large datasets strengthen the epidemiological evidence.

However, they do not change the fundamental principle:

Observational data must not be interpreted like a randomized clinical drug trial.


2026 | Meta-analysis on inflammation and oxidation markers

A systematic review and meta-analysis published in 2026 focused specifically on:

randomized controlled trials involving the daily consumption of extra virgin olive oil.

Included were:

23 randomized studies with a total of 1,138 participants.

Among other things, biomarkers such as the following were examined:

  • oxidized LDL
  • CRP
  • TNF alpha
  • IL 6
  • IL 10
  • IL 18

The evaluation found changes in various markers associated with the consumption of extra virgin olive oil.

However, the authors rated the certainty of the evidence depending on the endpoint as only:

moderate to very low.

This is a good example of a positive, but not limitless, scientific statement.

There are interesting signals from randomized studies.

But individual biomarkers are not the same as proof that a specific olive oil treats or prevents a disease.


Mediterranean diet | Olive oil is important, but not everything

Olive oil is closely linked to the Mediterranean diet.

However, the Mediterranean diet does not consist of:

a single food item.

It traditionally encompasses a dietary pattern with varying proportions of:

  • vegetables
  • fruit
  • legumes
  • whole grains
  • nuts
  • fish
  • olive oil
  • other regional foods

A 2024 meta-analysis on older adults included:

28 studies with a total of 679,259 participants.

In those studies, high adherence to a Mediterranean diet was associated with:

  • 23 percent lower all-cause mortality
  • 27 percent lower cardiovascular mortality

assessed.

Most of the included studies were observational.

The positive signal therefore concerns the entire dietary pattern and must not be attributed entirely to olive oil alone.


Why studies on the Mediterranean diet and studies on olive oil must be read separately

This distinction is central to me.

When a Mediterranean diet performs well in a study, various elements can contribute together.

These potentially include:

  • fat quality
  • more plant-based foods
  • fiber
  • fewer highly processed foods
  • fish and nuts
  • total calories and meal structure
  • other lifestyle factors

Olive oil can be an important part of this pattern.

But it would not be scientifically sound to automatically attribute every benefit of a Mediterranean diet to a single bottle of olive oil.


Is olive oil with many polyphenols automatically healthier?

I would not formulate it in such a sweeping way.

A high total polyphenol count can be an:

extraordinary analytical property.

Certain phenolic profiles may also meet the legal requirements of the EU claim.

But it does not automatically follow that:

The higher the total polyphenol count, the greater the health benefit for every person.

Why not?

Because the following can vary:

  • composition of the phenols
  • measurement method
  • amount consumed
  • overall diet
  • individual metabolic situation
  • quality and sensory profile of the specific oil

The strongest statement is therefore the one that the specific analytical result actually supports.


Oleocanthal | Sensorially fascinating, but no separate EU claim

Many people experience high-quality olive oil pure for the first time and are surprised.

It gets peppery in the throat.

Sometimes slightly.

Sometimes very distinctly.

A compound that is closely linked to this characteristic throat irritation is:

oleocanthal.

That is sensorially and analytically fascinating.

But:

There is no separate approved EU health claim for oleocanthal.

A high oleocanthal value may therefore not be automatically converted into a medical efficacy promise.

Quality remains important

When we talk about nutrition, the quality of the specific olive oil should not completely disappear from view.

A high-quality extra virgin olive oil should meet the corresponding chemical and sensory requirements.

For me, further questions arise with premium olive oil:

  • Where does it come from?
  • Which olive variety was used?
  • Which harvest is current?
  • How was it processed?
  • How does it taste?
  • What current analyses are available?
  • How transparent is the producer?

Health research and quality assessment are two different levels.

They can complement each other.

But they must not be confused with one another.


Can olive oil be used for cooking?

Yes.

Extra virgin olive oil does not have to be used exclusively cold.

It can be used for cold dishes as well as for numerous applications in warm cooking.

For example for:

  • searing
  • braising
  • baking
  • oven-baked dishes
  • vegetables
  • legumes
  • fish

A portion of the more sensitive aromatic substances and phenolic compounds can decrease with prolonged or intense heating.

This does not mean that extra virgin olive oil cannot be heated at all.

Anyone who wants to fully experience an oil with a strong character sensorially can additionally use some of it:

as a finish after cooking.

Is olive oil healthy? | Quantity is part of the answer

Olive oil is fat.

And fat is energy-dense.

One gram of fat provides approximately:

9 kilocalories.

Therefore, the question of a sensible amount only makes sense in the context of the entire diet.

More is not automatically better.

This also applies to very high-quality olive oil.

The:

20 g

from the European polyphenol claim are:

not a general medical dosage for every person.

They are a concrete condition of this approved claim.


20 g does not mean that olive oil has to be drunk pure in the morning

The conditions of the European claim do not say:

Olive oil must be taken in the morning.

They also do not say:

Olive oil must be drunk on an empty stomach.

And they do not say:

20 g must be consumed pure in one gulp.

Olive oil can be a completely normal part of meals.

For example:

  • over salad
  • over vegetables
  • with legumes
  • with fish
  • in warm dishes

Those who consciously taste premium olive oil pure do so primarily for:

sensory and culinary reasons.

On a commercial food platform, the question is not only:

What is scientifically interesting?

Equally important is:

Which health-related statement is approved for food?

For olive oil, two areas are particularly important.

Oleic acid and unsaturated fatty acids

Under the legal requirements, the approved statement regarding the maintenance of normal blood cholesterol levels may be used when replacing saturated fatty acids with unsaturated fatty acids.

Olive oil polyphenols

Under the legal requirements, the approved claim regarding the protection of blood lipids from oxidative stress may be used.

These two positive statements are significantly more robust for commercial communication than freely formulated statements like heart protection, anti-inflammatory, or protects against diseases.


What must not automatically become of studies?

A scientific publication can show an interesting connection or a positive effect of an examined nutritional intervention.

In advertising, this must not automatically become:

This specific olive oil prevents heart attacks.

Or:

This olive oil treats diabetes.

Or:

This olive oil cures inflammation.

Such statements would be a different level both scientifically and under food law.

Study result, approved health claim, and product advertising must remain separate.


Why laboratory values are nevertheless so valuable

The fact that a laboratory analysis does not prove a medical effect does not make it any less interesting.

On the contrary.

If an olive oil is described with special analytical properties, I want to be able to verify them as much as possible.

An analysis provides an objective level.

Sensory analysis provides a second level.

Origin and producer provide a third.

And personal use later provides a fourth.

That is exactly why I say:

Analysis without a human remains a number.

Experience without analysis remains an observation.

It becomes interesting when both levels are cleanly connected with each other.


How do I recognize high-quality olive oil?

Anyone interested in olive oil as part of their diet should also consider the quality of the specific bottle.

Among other things, I pay attention to:

  • the extra virgin olive oil grade
  • traceable origin
  • olive variety
  • producer
  • harvest or vintage
  • processing
  • sensory flawlessness
  • fruitiness
  • appropriate storage
  • current analyses, if working with special analytical values

None of these features alone decides everything.

Together, a much better quality image emerges.

If someone asks me today:

"Ayhan, is olive oil healthy?"

then I do not answer with a sweeping marketing sentence.

I would say:

Olive oil actually possesses properties that are nutritionally interesting and in some cases very well researched.

We have:

  • a characteristic fatty acid profile
  • approved positive EU statements regarding unsaturated fatty acids
  • an approved EU claim for suitable polyphenol-containing olive oils
  • large observational studies with favorable connections
  • randomized nutritional data from PREDIMED
  • further randomized studies on various biomarkers

That is a remarkable body of data for a traditional food.

And this positive evidence does not need to be downplayed.

But:

Not every bottle is the same.

Not every analysis is the same.

Not every olive oil automatically meets the requirements of a specific health claim.

And no single bottle replaces an overall balanced diet or medical therapy.

For me, this exact combination is decisive:

formulate strongly where the evidence is strong.

distinguish exactly where science and food law set limits.

That does not make olive oil any less fascinating.

It makes the story more credible.
